It belongs to the group of acylpiperazine opiates and was originally patented in the 1980s. Although it deviates structurally from fentanyl, it shows similar pharmacological activity.
Pharmacology and potential abuse
Preclinical studies have shown that 2-methyl-AP-237 has a high affinity for the μ-opioid receptor and has a stronger analgesic effect than morphine in animal studies. It has been found to completely replace the discriminatory irritant effects of morphine in rats and is about four times more potent. The effect can be blocked by opioid antagonists such as naloxone. Like other opioids, 2-methyl-AP-237 can lead to rapid development of tolerance and physical dependence.
Illegal distribution and health risks
In the US, 2-methyl-AP-237 was first identified in 2019 in drug sequestrations. Since then, the number of reported cases has increased, with at least ten deaths by the end of 2022. The substance is often offered in capsule form and varies in purity and dosage, which increases the risk of overdoses. In Germany, 2-methyl-AP-237 has also been detected in drug shipments, which is a cause for concern.
Legal classification
Due to its high risk of abuse and associated health risks, 2-methyl-AP-237 has been placed under national narcotics laws in several countries. In Germany, the substance was included in the Narcotics Act in 2019. At international level, it has also been classified as a hazardous substance by the World Health Organization (WHO).
